COVID-19 Patient/Visitor Protocol  

Testing Positive for Covid-19 Policy:
• If you are a patient and test positive for Covid-19, you must wait 10 days and be asymptomatic before returning.

Travel Policy:
• Fully vaccinated patients may return 5 days post travel if asymptomatic.
• Unvaccinated patients may return 10 days post travel if asymptomatic.

Patients Seeing a Provider:
• Adult patients may bring 1 caregiver or guest for support to their appointment with a provider.
• Pediatric patients under 18yo may be accompanied by both parents, but must be accompanied by one parent/guardian.

Patients Visiting Our Shot Room:
• Patients 18yo and older should come alone to their appointment.
• Patients under 18yo should be accompanied by only 1 parent.

General Office Information:
Due to the recent uptick in Covid-19 cases, we will not be allowing multiple children without appointments to come into our practice.

We also strongly encourage our patients that are able to be vaccinated against Covid-19 to do so.
